Each year, we come together as a network to reflect on our shared progress and recognize those health centers whose performance stood out across key value-based care metrics. Our Best in Class awards honor members that achieved the highest performance in 2024—and those who demonstrated the most significant improvement.
This year’s awards were presented at the May 2025 Value-Based Care General Membership Meeting, where we celebrated our collective impact and spotlighted members whose efforts are leading the way in improving outcomes and delivering high-quality care.
