For decades, Community Health Provider Alliance (CHPA) and Colorado Community Managed Care Network (CCMCN) have worked alongside one another to advance more efficient, high-quality healthcare delivery for all Coloradans. Earlier this year, these companies merged to streamline their strengths and enhance support for the community health centers and patients they serve. Today, a new chapter in Colorado’s healthcare industry begins with the public launch of the merged organization and its new name, Carina Health Network (Carina).

Through data insights, coaching, technology solutions, and value-based care programs, Carina empowers community health organizations to improve outcomes, increase efficiency, and deliver patient-centered care. The merger optimizes access to the technical expertise and resources necessary to create a more connected, efficient healthcare system that puts care teams and patients first.

The CCMCN and CHPA boards selected Jason Greer as the CEO of Carina. Greer has driven innovation in community health in Colorado for decades and is proud to lead the merged organization in this next chapter.
“The merger of CCMCN and CHPA is a demonstration of our commitment to performance improvement in the healthcare industry,” said Jason Greer. “Combining the strengths of these trusted organizations allows us to increase efficiencies for our providers, improve outcomes for the patients they serve and decrease the cost of care for the payers that provide their coverage. We look forward to building healthier communities through our network of community health centers.”
