Value-Based Coding Advisor

The value-based coding advisor (also posted as “risk adjustment and coding advisor”) supports coding accuracy, medical record documentation, quality gap closure, and provider education for the Community Health Centers (CHCs) participating in the Community Health Provider Alliance (CHPA) Accountable Care Organization (ACO). The personnel in this position will use their knowledge of risk adjustment coding, documentation guidelines, and report generation to assist in any coding and auditing-related functions determined by their direct supervisor or CHPA leadership.

Position Pay Range: $55,953 – $83,930

Responsibilities: The value-based coding advisor will interact with operational and clinical leadership to assist in the identification of Risk Adjustment/HCC coding opportunities and will provide targeted education to CHC providers, billers, coders, and support staff to support value-based contract initiatives.

  • Risk Adjustment/HCC Coding Support and Education
    • Educates providers and staff on coding regulations and changes as they pertain to risk adjustment and quality reporting to ensure compliance with federal and state regulations.
    • Assist the department, direct supervisor, and CHPA in the development of education tools related to risk adjustment/HCC coding and gap
    • Supports the creation of education that will train CHC providers, billers, coders, and support staff, as well as CHPA staff, for risk adjustment/HCC coding
    • Maintains a database with the results of all medical chart reviews performed, with the ability to report on progress and statistics on coding
  • Pre-Visit Planning (PVP)
    • Performs weekly Pre-Visit Planning reviews for assigned CHCs and will query providers or other identified team members to further value-based contract initiatives including coding recommendations based on internal and external medical records, review of payer portals and suspected conditions, and review of care gap and clinical documentation.
  • CHC Support
    • Holds monthly meetings with identified coding champions, provide education and training to CHC providers, billers, coders, and support staff in proper coding guidelines; and documentation education based on PVP observations and monthly
    • Provides monthly chart reviews of randomly selected patients and providers participating in the Pre-Visit Planning (PVP) program to give feedback on missed opportunities and
  • Gap Closure Success
    • Reviews patient charts to identify areas for quality gap closures and provide compliant documentation to appropriate payers resulting in gap closures for assigned
    • Ensures that providers understand CPT II coding for quality gap closure and
  • CHPA Success
    • Assist in all CHPA dashboard and scorecard initiatives to improve performance outcomes.
    • Assist supervisor in implementing best practices, policies, and procedures to increase support services to the CHCs we serve
    • Assist in developing and sharing guidelines/best practices with internal risk adjustment coders to help improve coding documentation techniques for our members
    • Helps with special projects within the Risk Adjustment Department

Qualifications: To achieve high-quality outcomes the value-based coding advisor should be knowledgeable about HEDIS/STARs measures and guidelines, CMS Hierarchical Condition Category (HCC) coding guidelines, clinical documentation standards, and education/training skills.

  • High School diploma or equivalent
  • Minimum 2 years coding experience
  • The American Academy of Professional Coders (AAPC) Certified Risk Adjustment Coder (CRC) certification is required; Certified Professional Coder (CPC) Certification will be considered with Risk Adjustment/HCC Coding experience and willingness to obtain CRC within 1 year of employment
  • Risk Adjustment experience required
  • Experience with clinic billing and coding required
  • Knowledge of several EHR systems preferred (ECW, Athena, Greenway Intergy, Epic)
  • Clinical background preferred
  • Strong knowledge of CMS coding and quality guidelines
  • Strong knowledge of PowerPoint, Excel, and Microsoft Word with the ability to manipulate basic information and data required for preparing reports and delivering training
  • Exceptional interpersonal, public speaking, and presentation skills to deliver training and education are preferred
  • Ability to facilitate group discussions that challenge participants and promote discussion of new approaches and solutions based on data and value-based care initiatives
  • Ability to adapt to changing initiatives and priorities based on the needs of CHPA and CHC success

Pharmacy Tech Medication Adherence Specialist

The Pharmacy Tech Medication Adherence Specialist (Pharm Tech) supports CHPA members in meeting Healthcare Effectiveness Data and Information Set (HEDIS) and Medicare Star Ratings medication adherence and utilization measure goals. Members include Community Health Centers (CHCs), also known as Federally Qualified Health Centers (FQHCs), participating in the Accountable Care Organization. The personnel in this position will use their knowledge of pharmacy workflows, health coaching, and insurance plans to assist in development and implementation of supports related to medication adherence/utilization and as determined by their direct supervisor or CHPA leadership.

Report To: Chief Operating Officer

Date: March 2024

Hiring Range: $20-25 per hour

Responsibilities:

  • The Pharm Tech will interact with operational and clinical leadership to assist in the identification of medication adherence/utilization opportunities and will provide targeted support to CHCs to improve outcomes in CHPA’s value-based contract initiatives.
  • Medication adherence/utilization support
    • Improve medication adherence rates by identifying barriers that prevent patients from taking medication regimens appropriately and work with care team members to help patients be successful in managing their health.
    • Telephonic and electronic outreach to patients based on internal and payer reports that identify those who may need additional help in managing medications and getting timely refills.
    • Coordinate with members of the patient care team, including providers, pharmacists, nurses, and patient care coordinators to address patient needs related to medication use.
    • With a strong knowledge of pharmacy workflows, insurance plans, and available resources, help patients navigate the complexities of obtaining prescription medications and refills.
  • CHC Support
    • Assist the department, direct supervisor and CHPA in the development of education tools related to Medication Adherence/Utilization.
    • Support and assist the Pharmacist in the creation of education that will train members of the patient care team as well as CHPA staff, for medication adherence/utilization.
    • Maintains a database with the results of outreach calls to patients and report on progress and statistics on the measures.
    • In collaboration with the Pharmacist, provides monthly or ad hoc feedback on opportunities to improve medication adherence/utilization.
  • CHPA Success
    • Assist in all CHPA dashboard and scorecard initiatives to improve performance outcomes.
    • Assist supervisor in implementing best practices, policies, and procedures to increase support services to the CHCs served.

Qualifications:

To achieve high-quality outcomes the Pharmacy Tech is to have knowledge of HEDIS/Stars measures and guidelines, clinical documentation standards, and have the following education/training and skills:

  • High School diploma or equivalent
  • Certified Pharmacy Technician (CPhT) preferred with at least 3–5 years of community or retail pharmacy technician experience; a Medical Assistant or LPN will be considered with experience in HEDIS/Stars measures, specifically medication adherence/utilization measures
  • Experience with HEDIS medication adherence and utilization measures
  • Knowledge of several EHR systems preferred
  • Experience with Microsoft Office including PowerPoint, Excel, and Microsoft word with the ability to manipulate basic information and data required for preparing reports and delivering training
  • Exceptional interpersonal, public speaking, and presentation skills to deliver training and education preferred
  • Exceptional customer service skills
  • Ability to adapt to changing initiatives and priorities based on the needs of CHPA and the CHC’s success

Practice Transformation Coach

The Practice Transformation Coach is accountable for providing transformation support to member Community Health Centers (CHCs) resulting in improved quality outcomes and network performance in CHPA’s VBC programs. The Coach has a defined role, working in an advisory/coaching capacity to support CHPA CHCs and assist in creating and implementing individualized CHC CHPA Support Plans to optimize performance. The Coach utilizes an evidence-based approach to facilitate quality improvement (QI) methodology and tools within CHCs, including assessments, reporting measures, and facilitating meetings with practice-specific QI teams working towards achieving measurable outcomes. This position requires traveling to CHCs and preference will be given to qualified applicants residing in northern Colorado or the western slope of Colorado. Position Salary Range: $75,000 – $90,000k

Responsibilities:

  • Develops, implements, and monitors individualized CHC CHPA Support Plans, targeting practice transformation activities to increase success in CHPA’s value-based contracts.
  • Serves as a liaison, teacher, coach, and facilitator for practice transformation activities for CHCs participating in the network.
  • Obtains, aggregates, and analyzes quality metrics and reports for care opportunities; identifies opportunities to improve cost, quality, and the patient experience; and prioritizes opportunities in individualized CHC CHPA Support Plans.
  • Participates in the development and implementation of community learning forums.
  • Provides onsite and telephonic CHC coaching visits using a standardized approach and demonstrates the ability to effectively lead and facilitate improvement teams towards measurable outcomes.
  • Maintains a supportive and collegial role with the primary care team and collaborates to meet provider and practice needs.
  • Conducts ongoing CHC assessments to learn more about practice demographics, daily operations, and current workflows.
  • Supports the efforts of providers and practice teams on interventions resulting in cost of care savings and improved health outcomes for patients.
  • Provides education for practices to develop expertise with Annual Wellness Visits (AWVs), metrics, and data review for quality improvement.
  • Works with CHPA leadership on communication to physicians regarding available patient care programs through the VBC programs – both written/email communication and visits to provider meetings, as necessary.
  • Reviews reports from payers and participates with leadership in the interpretation of the reports and actions needed.
  • Shadows, educates, and/or trains physicians and medical staff as necessary in clinical workflows and coordination of care to improve clinical quality and achieve VBC goals. Test and pilot new VBC initiatives and work with CHPA leadership on refining processes, helping achieve contractual metrics.
  • Other duties as assigned.

Qualifications:

  • 2-3 years of experience in primary care, ambulatory care, or another related field. Preference to individuals who have experience working at CHCs, also known as Federally Qualified Health Centers (FQHCs)
  • Preference will be given for experience completing Annual Wellness Visits (AWVs).
  • Preference will be given for strong background in one or more of the following: VBC programs, quality or process improvement, data analysis, healthcare recognition/accreditation, and/or change management.
  • Experience in population health management, community health, case management, disease management, quality improvement, and/or motivational interviewing and active listening.
  • Experience leading Lean, Model for Improvement, or other quality/performance improvement methodology.
  • Experience in ACO/Value-Based Contracts or Managed Care Contracts.
  • Experience in public speaking centered around medical practice education to include training and presenting in small or larger group situations.
  • Excellent oral communication skills both in-person and telephonically, to groups and individuals.
  • Excellent written communication skills, and knowledge of Microsoft Office tools.
  • Strong critical thinking, analytic, strategic, and innovative skills.
  • Ability to travel within the state of Colorado – 25% travel within the state of Colorado, likely the western slope.
  • A valid unrestricted U.S. driver’s license, reliable personal vehicle, and insurance is required.
  • Proof of full COVID-19 vaccination may be required for employment.

Community Health Provider Alliance (CHPA) is an Accountable Care Organization (ACO) that supports Community Health Centers in Colorado by being the expert partner they need to manage value-based care contracts and more. We achieve strategic goals by working within four pillars of success.

To increase long-term sustainability CHPA has partnered with various payers across the state and nation, entering into various value-based contracts to set quality goals. CHPA helps CHCs understand and reach measure goals to achieve savings.

CHPA’s risk adjustment and quality team provides pre-visit planning and education for providers, nurses, coders, and billers. Risk adjustment accuracy ensures a complete and accurate picture of patient populations, supporting continuity of care and a reduction in claim denials for our member Community Health Centers.

The practice transformation team provides members with various touchpoints to help refine and improve primary care delivery to advance quality improvement, patient-centered care, and characteristics of high-performing primary care.

CHPA’s data team produces monthly and quarterly performance reports to give members continuous feedback.
