Diabetes is one of the most common and costly chronic diseases in the U.S., affecting over 38 million adults — with nearly 1 in 4 unaware they have it. Another 97 million have prediabetes, placing them at high risk. The financial toll is steep: $413 billion annually, accounting for 1 in every 4 healthcare dollars. The burden falls hardest on communities already facing barriers to care.

Hypertension, or high blood pressure, affects nearly half of U.S. adults — yet only 1 in 4 have it under control. Often symptomless, it’s known as the “silent killer” and contributes to hundreds of thousands of deaths each year from heart disease, stroke, and kidney failure. It carries an annual cost of $131 billion and disproportionately affects people of color, rural populations, and those with lower incomes.

Depression is a leading and growing mental health concern, now affecting nearly 1 in 3 Americans. Rates have surged in recent years, especially among young adults and communities of color. In Colorado, over 40% of adults report symptoms of anxiety or depression, and 1 in 5 are unable to access the care they need — due to stigma, cost, and systemic barriers.
At Carina Health Network, we’re working to close that gap. Over the past five years, we’ve improved depression screening and follow-up by 8.5% across all populations in our network.
We help shift care away from unnecessary emergency visits by supporting more timely, coordinated, and trusted care within local communities.

Sometimes, the emergency department feels like the only option — especially after a long day of work, cooking dinner, and caring for a family. It’s open, it’s familiar, and it’s accessible when other care isn’t. We partner with community health organizations to change that. By supporting access to timely, trusted care through local providers, we help patients get the right care, at the right time — in the right place. Since 2019, this work has contributed to a 26.2% decrease in emergency department visits.
Carina Health Network is reducing costs and reinvesting in care. Our value-based care approach delivers better health and financial sustainability.
In value-based care, Total Cost of Care is key. It includes everything from hospital stays and prescription drugs to preventive visits and long-term care. As national Medicare spending now exceeds $12,000 per beneficiary annually, containing costs while improving outcomes has never been more urgent.
