Data makes up the backbone of high-performing practices. Data from meaningful measures is published quickly, precisely, and routinely providing rapid feedback on practice performance.

The keys to success, as outlined in Dr. Thomas Bodenheimer’s publications on the building blocks for high-performing primary care practices are:

  1. Prioritize data collection and reporting by finding the time to collect, clean, and summarize your data. Develop a regular cadence for reviewing the data with key personnel.
  2. Be strategic when choosing which metrics to tackle. While CHPA hopes you can address each of the metrics on our dashboard, we understand that you may only have the resources to tackle a few.
  3. Identify organizational goals to move from data to action. Post action steps, timelines, and outcomes in a public place.
  4. Analyze variation by drilling down to individual pieces of the process required to meet a metric. For example, a drop in annual wellness visits may be an access problem, a scheduling problem, a provider problem, or a billing problem.
