Patient Name Must Match What’s Listed on Insurance Card

Recent work with many CHC’s has identified a challenge. We frequently find patients using multiple versions of a name. Here is an example from our CHPA Team:

  • My legal name (on my government issued identification card): Betty Jane Schultz Dempsey
  • Preferred: is BJ Dempsey
  • Name on the health insurance card: Betty Dempsey

For medical claims to process correctly, the name on the claim must directly match the name on the insurance card. In this case, “Betty Dempsey” is correct. This includes initials and suffixes, such as Jr., Sr.

The impact of improperly submitted claims affects:

  • Revenue
  • Attribution
  • Performance on quality metrics
  • Performance on CHPA distribution score card

Additional reminders in this area are the importance of scanning or copying the identification card and the insurance card and accurately recording the payer type in the demographic section of the record.
