Thoughts from the CEO, May 2022

By: Kerry Cogan

Happy May!  I hope everyone is having a great Spring. I want to start off by congratulating three members of the CHPA team for their recent accomplishments!

  • Christine Smidt passing her CRC (Certified Risk Coder) exam
  • Gary Romero and Loretta Lathon each passing their CDEO (Certified Documentation Expert Outpatient) exam

These competencies leverage the knowledge and experience that CHPA can provide to the CHCs and is a great addition to our expertise. The CDEO credential puts a whole new light on correct documentation to support suggested codes for the providers. Education is the key to move the needle on improving our quality as a network!

We held our General Membership meeting On May 5th. There was a great turnout and we really appreciate everyone taking the time to attend the virtual event. During the meeting, we went through the 2021 final performance data as a network and showed specific CHC performance on a variety of the clinical quality measures we are held to in Value-Based Care (VBC). It is critical that we increase our overall quality and we can accomplish this by increasing our volume of Annual Wellness Visits (AWV), and by improving HCC Coding and Coding Recapture rates.  The CHPA team is working hard to provide additional resources to all CHCs, including educational articles on coding, etc. in this and each edition of the newsletter.

I also want to personally congratulate Summit Community Care Clinic for surpassing the Annual Wellness Visit (AWV) goal of 20% and winning ‘Best in Show’ at our General Membership meeting! Summit and Dr. Cowie’s team have consistently focused on AWVs and have an impressive model focused on these visits. Other high performers that we recognized at the meeting include:

  • Annual Wellness Visits – Most Improved: High Plains Community Health Center
  • HCC Recapture Rate – Honorable Mentions: Pueblo Community Health Center and Marillac Health;
  • HCC Recapture Rate – Most Improved: Northwest Colorado Health
  • ED Utilization – Honorable Mentions: Axis Health Systems, High Plains, Northwest, Sheridan Health Services, Summit and Valley-Wide Health Systems.

Congratulations to all the care teams and CHCs in achieving these recognitions. AWVs, HCCs and ED utilization all are core components to succeeding in VBC. The six CHCs recognized for ED Utilization were all under the MSSP national average of 567/k across all ACOs. This is impressive and directly contributes to lowering overall healthcare expenses and hospital utilization. Way to go CHCs!

To further emphasize and introduce the concepts of VBC to the CHCs, please join us for the two-part series ‘Operational Excellence: Preparing Teams to Succeed in Value Based Care’ that is coming up on 5/19 and 5/26.  Part one on 5/19 will cover ‘The Move to VBC and the Impact of APMs’ and part two on 5/26 will cover ‘Building Blocks and Best Practices’.  If you are missing the calendar hold for this valuable education series, please contact caitlyn@chpanetwork.com. This should be a highly informative session with valuable insights from Dr. Dudley, Brandi Apodaca, and other experts on the core components of delivering high-quality VBC. We highly encourage you all to attend!

Thanks again for all you do to serve our patients and communities across Colorado.
