Who We Are
Community Health Provider Alliance (CHPA) is a nonprofit Accountable Care Organization (ACO) leading the transition to value-based care. CHPA was formed in 2014 around a statewide integrated network of Colorado Federally Qualified Health Centers (FQHCs), also known as Community Health Centers (CHCs), that offer integrated primary care services to patients regardless of their ability to pay.
